COSMETIC EYELID SURGERY (BLEPHAROPLASTY)

Cosmetic eyelid surgery is the second-most performed cosmetic surgery after rhinoplasty (cosmetic nose surgery) in Turkey. Although it is mostly conducted for cosmetic reasons, functional reasons may also be involved in eyelid surgeries. An eyelid is comprised of two parts: upper and lower eyelids. Upper and lower eyelid surgery can be combined or performed separately.

LOWER EYELID SURGERY

Lower eyelid surgery is used to treat problems caused by genetic factors, aging, or previous injuries, such as under-eye bags, sagging, and drooping eyelids. Before the surgery, patients undergo a thorough eye examination, during which their medical history is taken, and problems are discussed. Then, in a general physical examination carried out for the surgery, the fat tissue and muscles in the patient’s eyelids are carefully inspected and evaluated.

Since lower eyelid surgery is usually handled together with sagging cheeks, the result is achieved by making aesthetic interventions to the centre of the face. Therefore, the surgery is performed under general anaesthesia. During the procedure, an incision is made just below the eyelashes, and the eyelid skin and muscle are lifted together. The fat tissue revealed here is disseminated under the eye, and excess skin and muscle tissue are removed. However, the fat tissue is not intervened if it is not too large. In this way, the procedure is completed by removing the under-eye bags.

The surgery takes about 2-4 hours. The stiches (sutures) under the eyelashes are taken out after 5 days or left until they are naturally dissolved. Postoperative oedema and bruises are healed completely within 10 days. In the first 2 weeks following the surgery, patients should protect the surgical site from the sun, and wear sunglasses outside.

Lower eyelid surgery yields long-lasting results. Although under-eye bags may loosen after some time due to ageing, they never return to their preoperative state.

UPPER EYELID SURGERY

Some people may experience upper eyelid sagging due to genetic factors or ageing that obstructs vision and makes them look older. In those cases, the excess skin of the upper eyelid is surgically removed. As a result, a better vision and a younger and more dynamic look are attained.

Before the surgery, like in lower eyelid surgery, patients undergo a series of required tests and a thorough eye examination, during which their medical history is taken, and problems are discussed. The surgery is performed under either local or general anaesthesia and takes between 1 and 2 hours if no additional procedures are going to be performed. After the operation, incisions are covered with small adhesive strips. These are later removed, and the wounds are cared for on the 5th or 6th day by our doctors. Postoperative swellings and bruising are normal and usually subside after the first 3 days. Patients are expected to fully recover within 10 days.

Patients should not touch or rub their eyes and the surgical site in the first 36 hours. LIP-LIFT SURGERY

Lip-lift surgery, or lip-lifting, is a surgical procedure performed to reduce the distance between the upper lip and the nose, lift the drooping upper lip, and enhance lip proportion. During the procedure, an incision is made in the skin right under the nose that extends towards the upper lip. A small patch of excess skin is removed, and the remaining area is carefully stitched back together. According to the consensus among many cosmetic surgeons, the ideal length between the nose and the lip is around 12–15 millimetres. However, an amount of lip fillers can be injected to into the lips of patients who desire to have plumper lips during the procedure.

Patients who do not wish to change their lip proportion but would like their lips to look lifted or their teeth to be more visible when they smile can also benefit from this procedure. Moreover, lip-lift surgery, which is becoming increasingly widespread in Turkey, is the right procedure for you if you regularly receive lip fillers and desire to have permanently plumper lips.

The procedure is usually performed under local anaesthesia and takes about 45 minutes. Patients must stop taking blood thinners and drinking herbal tea blends 15 days before the procedure. Patients can return to their daily lives soon after the surgery, but they are advised not to engage in strenuous physical activity for a few days and sleep in the prone (face down) position. Swelling around the surgical site is expected in the first 3 days after the surgery. In the meantime, applying a cold compress to the lips would help with the swelling and pain. It takes roughly a week for the surgical site to heal completely, and it is normal to experience numbness and mild discomfort around the lips for some time, particularly while laughing. Sutures (stitches) are removed 5–7 days after the surgery. In addition, a pink, faint line of incision mark will be left after the lip-lift procedure; however, it will gradually disappear.

THREAD LIFT (FRENCH LIFT)

Thread lift is one of the alternative non-surgical facelift methods. Over time, facial muscles lose their strength against gravity, become sagging and loose, and cause wrinkles. This produces dents, particularly around the nose and mouth, making the person look old, tired, and unhappy. It has become easier to erase this wearing effect of ageing with methods such as thread lift. On what parts of the face can the thread lift be applied?

After disinfecting the treatment area, numbing cream is applied to it to provide local anaesthesia. In facial procedures, the attachment area where the threads are inserted is usually above the ears. The procedure typically takes about 45 minutes, and the results are visible immediately. There are two types of threads used in a thread lift procedure: permanent threads and temporary (dissolvable) threads. Permanent threads are more long-lasting and endure around 5-6 years while temporary (dissolvable) threads only last for 2 years.

It is normal to have puffiness, redness, swelling, and bruising on the face following the procedure. It is advised to avoid sleeping in the prone (face-down) position for a while and doing strenuous physical activity for a month. It is also important not to have dental treatment for several weeks and to protect the face from direct sunlight.

If desired, the thread lift procedure can be repeated after the effects of the previous procedure have worn off. Moreover, the permanent or temporary (dissolvable) threads can be removed if the patient is not satisfied with the result or changes their mind.

COSMETIC EAR SURGERY (OTOPLASTY)

While the ears are the organs responsible for hearing, at the same time, the part of the ear called the “outer ear,” or “pinna,” is a defining factor in the aesthetics of the face and head. Anatomic differences, such as outer ear deformities, large outer ears, missing portions of ear cartilage or excessively large and deep “concha” of the outer ear, and outer ears protruding from the head by a large angle are referred to as “lop” ear.

The development of the outer ear is typically completed by the age of 4-5, although the process may continue up until around age 10. As a result, cosmetic ear surgery can be performed on individuals as young as 4 years old, but it is generally recommended that this procedure be performed before children reach school age. This surgery helps to minimize any potential negative impacts on children’s psychology and socialisation, such as introversion or shyness, that may arise from having protruding ears.

Otoplasty surgery is performed on adults under local anaesthesia, while children and adolescents are placed under general anaesthesia. Before the procedure, a thorough examination is conducted to determine the extent of the problem. This allows for a surgical procedure to be performed by an experienced surgeon that will correct all existing problems, resulting in highly successful outcomes.

Cosmetic ear surgery is a straightforward procedure that yields definite results and typically takes about 1-1.5 hours. The surgery involves making an incision behind the ear and performing a procedure based on the underlying cause of the problem, usually reshaping the ear cartilage to achieve a more natural shape. Afterwards, the ear is repositioned backward. Special bandages that resemble a headband are applied to the ear after the surgery and should be worn for 1-2 weeks.

Otoplasty does not require hospitalization after surgery. In this operation, performed in Istanbul's leading hospitals, the patient is discharged on the same day. The patient can take a shower 1-2 days after the surgery. Normal activities can be resumed within a week at the latest. Full recovery usually takes 1-2 weeks. During this process, it is important to protect the ear from external factors and to follow the recommendations of the surgeon who performed the operation.
